Oil pump drive bolt failure

Unless I am seeing digital "artifacts" it sure looks like the 2nd and 3rd threads on the head side of the bolt have been squashed nearly into one(or folded on top of each other). I only say that as I examined closely my tranny drain bolt that someone had overtightened to failure, looked similar. Sure looks like over an over torque leading to failure.
