1- your bad idle problem: I would think about a carbonized head+valves but you said you went thru that some time before. Bad injector?? (really not common) In my case, my car always had a rough idle problem and all injectors are ok, + ignition system. But my compression is really too high so it is my combustion chamber which is carbon coated.

2- The "ok when idle but struggles when touching the gas pedal"... I think it could be related to a bad Air Mass Meter... I hope for you that i'm wrong$$$! Try to disconnect the sensor(ENGINE OFF) and restart with the unplugged AMM. If your Idle is better without the AMM, bad news... it is probably bad.
