I got most of the linkage parts from TWM Induction, http://www.twminduction.com/.
Here's a photo of the installation, though the linkage isn't really clear (you can see the cable, though).
I have some AutoCAD drawings of the parts I made, and I can get better pictures; e-mail me if you're interested.
