Theoretically, there is zero or some negative pressure because the engine itself should give an average of zero but the intake vacuum will make it go negative. But this assumes no blow-by past the pistons or valve stems and you always get this on an older engine producing positive pressure. So, in my opinion, pressure on most engines will be a bit positive. However, I would welcome other opinions.
