A loose cam sprocket? If it sounds louder towards the front of the engine. A mechanics stethescope may help you pin point the location. I would also do a good visual inspection of all rotating components while running.

If an automatic a loose or broken flexplate.

Next; drain the oil check for metallic debris.

5000 miles of city driving is pretty severe service IMO, I would do an oil analysis to be certain the oil is not too contaminated.
