Multiple Alternator Failures

Before grounding the D+, DISCONNECT the wire!
What are you grounding your meter to? Be sure you are geting true readings here.
I ground to the battery - for all tests. Battery minus is the lowest point in the system, voltage wise, and is reference for all tests.
Ground at battery and test at alternator ground, alternator +, D+ and Battery plus.
If readings are as you say then pull and check the brushes on the voltage Reg.
If OK, take whole alt. to electrical repair shop, they should give you a free alt. test.
If alt is OK in their test I would check D+ with wire off alt, then with wire connected. Should be the same.
