If the car has a MT, had an aftermarket belt installed w/o the OEM plastic tensioner "retainer clip" AND the car was on a hill, in gear.......I've heard that the tensioner could leak down (depress) and thus when you start the engine, the belt can jump. That was the purpose of a $.02 plastic retainer clip that was even a recall (or campaign) at one time but has been included ever since in all OEM Volvo T-belt boxes with the belt and sticker.
An aftermarket t-belt wouldn't have that part. Just another possible reason here but trying to pin fault on the independant installer will be difficult.
