I decided to start a new thread on my '89 240 as I've discovered a significant new clue.
Here's the rundown:
The engine is running very rich (gas in the oil rich) and rough but no codes. This started suddenly after my wife ran over a branch but I don't see any connection.
I swapped injectors, MAFS, ECU, plug wires, FPR, & oxygen sensor.
I disconnected cold start injector, verified coolant temp sensor reading @ ECU, routed fuel return line to jug in case it is plugged.
Very recently the primary fuel pump and filter were replaced, oil changed, throttle body cleaned, all connections triple checked.
I have now determned that plugs 1, 2, & 4 are black with soot while #3 is dry.
Spark on 3 but no fuel, meanwhile 1, 2, & 4 are very rich!
How does this happen?
