Timing belt riding forward - then breaking -aft head gasket replace -HELP

I need some good advice. If it is in the FAQ's -tell me. I will dig deeper. This is for my 92 745T.

Replaced the head gasket. All back together - cranks (misses a bit). 1st issue - timing belt moving forward - then notice second issue - oil coming from the seal area.

Pull the top timing belt cover - note that belt is not at TDC (it was before). Slide belt off - NO SEAL. No wonder it was leaking oil!

Push seal in place. Slide timing belt back in place. Use the tension of the belt - with a rachet on the lower pully - to tighten back up top cam gear - belt breaks.

Questions:
Why did it ride forward? Just the belt starting to go bad ?? Or are we not setting the tensioner right? (we just tightened the bolt after the belt took up the slack) Or did us using the belt to hold tension to tighten top cam gear break the belt prematurely? Juat want to not make the same mistakes again.
