Remove a plug or two and see if they are dark or light. Dark is rich, light is lean. At 16 mpg probably dark. A rich condition.

An O2 sensor not up to par would (should) set a code.

Cold start injector?? Plugged or restricted vacuum line to Fuel Pressure Regulator or fuel return line to tank. Did you run over something that bounced around under the auto?? Rare but it has happened.
