If it's the axle threads and nut, then I'd be a bit cautious. The cotter pin is not enough to restrain a loose castellated nut! I wonder if the hold down pins or clips may have rusted, essentially allowing the brake shoes to float. Regardless of the actual problem, I suggest you rent a wheel puller to find out exactly what's making that noise.
