I've always unscrewed it simply because it's so easy to do -- requires a 10mm wrench, or there abouts.
I also use drill bits of progressively larger sizes to clean out severely "gunked" nipples once I have them removed. You turn the bit with your hand, and it sort of chisels the hardened-on crap away -- it's very effective.
